Patient Information

Our office accepts almost all dental and medical insurances. We are a contracting provider with Delta Dental, Blue Cross and United ConCordia. We will code your procedures and submit them to your insurance, and try to obtain the maximum benefit to you that is allowed by your particular plan. We also will preauthorize the procedures to be done if you wish, but please be advised this may take 2-3 weeks to receive the information back from your insurance company. The amount that is not covered by your insurance is due at the time of surgery.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but that must be done prior to the actual procedure.
